Who is Formly for?
Start with 1 Code for one editor who needs branded forms, logic and Stripe payments. Move to 2 Codes if you need partial responses, duplicate-response prevention, drop-off reporting or up to 3 users. Select higher levels for real seat and storage requirements; buying them solely for AI credits is hard to justify without the separate top-up price.
Suitable for
Small teams using branded quizzes, surveys and payment forms below fair-use thresholds.
Consider another option if
A service requiring unrestricted high-volume responses, a custom sender domain or confirmed MCP access.

What comes with the lifetime deal?
$99 once; stack up to five codes
One-time lifetime allowance; manual form creation stays unlimited
Unlimited advertised responses, subject to published fair-use conditions
2 Codes and above; automatic reminder emails were still in development in the October answer
Future Gold Plan updates included
Redeem codes within 60 days; the AppSumo listing gives 60 days to request a refund.

Costs and limits to account for
- AI credits are one-time; extra bundles require a support quote.
- Partial responses and refill links start at 2 Codes.
- Unlimited responses remain subject to the vendor fair-use policy.
FEATURE COMPARISON
Features, limits and paid extras
“Included” and “unlimited” can mean different things. This checklist separates full access, usage limits, add-ons and features that are not confirmed.
6 fully included · 5 limited · 0 add-on · 0 not included · 1 not confirmed. Coverage is not a quality score.
Features are grouped by documented coverage. “Not confirmed” means the evidence is incomplete, not that a feature is absent.
Check each product’s plan notes for tier limits and additional costs. Feature coverage is not a quality rating.
USE CASES
What problems does Formly solve?
Collect a registration fee with the answers needed to fulfil it
Separate forms and payment links leave staff matching responses to payments manually.
How to use it
- Confirm that the business can use Stripe, then connect the correct account.
- Create the registration fields and calculate the price from the chosen options.
- Add the payment question and a confirmation; review the payment-status field in the resulting records.
What this helps you do: The intended result is one submission containing the selected options, calculated amount and payment outcome.
Limitations: Processor fees remain payable. Complex calculations need checking.
Show a useful quiz result instead of a generic thank-you page
A lead-generation quiz asks several questions but gives every respondent the same answer.
How to use it
- Define the result categories and scoring rules before building the questions.
- Map answers to scores or outcomes and show the matching result with an explanatory chart.
- Send completed responses to the follow-up system, keeping incomplete entries distinct.
What this helps you do: A respondent can receive a result connected to their answers, while the business receives more useful qualification information.
Limitations: AI-generated questions and scoring require review. Charts are a presentation feature, not evidence that the assessment is valid.
Find where a long intake form loses people
A total response count does not reveal which questions make visitors abandon a form.
How to use it
- Use at least 2 Codes for partial responses and advanced drop-off reporting.
- Review the incomplete entries and the question where people leave.
- Simplify unnecessary questions and use a refill link where a follow-up is appropriate.
What this helps you do: The workflow can identify changes to investigate rather than merely counting completed submissions.
Limitations: An observed drop-off is not proof of its cause. Automated reminders remain unconfirmed.
BUYER FEEDBACK
What buyers say
Buyer experiences can vary with the plan, product version and workflow.
Useful logic, partial responses and support
One buyer described using partial responses alongside completed poll entries, while others discussed assessments and helpful support. These are individual reports; the reviewed tier is not always stated.
Setup effort and custom domains
A July 2026 reviewer needed support to complete a calculator/payment workflow. In the one-taco sample, an April reviewer reported a custom-domain failure; the founder said the issue appeared specific to that domain and was still being investigated. This is not a reproduced fault or evidence of an outage across all accounts.
Older feature complaints need current tier context
A March 2026 critical review cited missing duplicate-response prevention and incomplete white-labeling. The current plan table now includes duplicate-response prevention from 2 Codes. The review should not be used to claim it is absent everywhere; custom email sender domains remain a separate limitation.

BEFORE YOU BUY
Formly lifetime deal FAQs
Do Formly AI credits renew each month?
No. The founder identifies them as one-time lifetime credits. Manual form creation remains unlimited, and additional AI bundles are available through support at a price not stated in the reviewed answer.
Which tier includes partial responses?
2 Codes and above. The same founder answer includes refill links from that tier. Automatic reminder emails for unfinished forms were still in development.
Which tier is suitable for a three-person team?
2 Codes includes 3 users. Choose a higher level only if domains, storage or another stated limit also requires it. Respondents filling out public forms are not the same as account users.
Does unlimited mean there are no usage restrictions?
No. The vendor publishes fair-use guidelines covering sustained monthly volume, burst traffic and resource-heavy forms. A higher code level does not establish an exemption.
Are payment processing fees included?
No. Formly says it charges no extra transaction fee, but Stripe fees still apply. Confirm processor availability and the relevant fee schedule for your business.
Can I remove branding on 1 Code?
The plan includes branding removal and your own logo from 1 Code. Do not treat that as a promise of a custom email sender domain or complete agency white-labeling.
Can I add more codes later?
The deal allows stacking up to five codes and requires redemption within 60 days of purchase. That does not guarantee future code availability, lifetime add-on pricing or a downgrade process after redemption.
